(ATTENTION PIMA COACHES AND STUDENT-ATHLETES): Sports Injury Management is having pre-participation physicals on  FRIDAY JULY 20TH at Dr. Hardy's office. This will allow any of your athletes to receive a free physical by the team physicians prior to the first day of practice on Aug. 1st.  This will be the ONLY physical date offered this year.  As requirement by the NJCAA, all student athletes must have a completed physical on file with the SIM staff prior to participation in any team practices.  If an athlete does not attend this date they will be required to get one on their own (which will cost). ALL PHYSICALS MUST BE SIGNED BY AN MD or DO (please make sure that your athletes know this if they are getting one on their own). Physicals will be approximately 8 am to 1 pm.

*Student-Athletes should wear shorts/T-shirts and bring their medical insurance information cards

*As per Dr. request, female student-athletes should wear a sports bra or tak top to allow for shoulder examination.

THE ANNUAL ATHLETIC PRE-PARTICIPATION MEDICAL SCREENING IS AN ELIGIBILITY REQUIREMENT OF THE NJCAA AND MUST BE COMPLETED PRIOR TO A STUDENT-ATHLETE BEING ALLOWED TO PARTICIPATE IN TEAM PRACTICE ACTIVITIES.  STUDENT-ATHLETES WHO WISH TO HAVE THEIR EXAMINATION DONE BY THEIR FAMILY PHYSICIAN MUST USE THE PIMA COMMUNITY COLLEGE PRE-PARTICIPATION ATHLETIC SCREENING /MEDICAL HISTORY QUESTIONNAIRE FORM.  THIS FORM MAY BE OBTAINED FROM THE ATHLETIC TRAINER OR THE OFFICE OF INTERCOLLEGIATE ATHLETICS.  STUDENT-ATHLETES WHO FAIL TO REPORT ON THIS DATE WILL BE HELD RESPONSIBLE FOR OBTAINING THEIR EXAMINATION.  SOME TEAMS WILL REQUIRE A SPLIT SQUAD FORMAT TO ALLOW A MORE EFFICIENT PROCESSING OF THE TEAM PARTICIPANTS.   IF YOU HAVE ANY QUESTIONS REGARDING THIS MATTER, PLEASE CALL THE SPORTS INJURY MANAGEMENT OFFICE AT 206-6012.

Please encourage your athletes to come that day if they haven't already got a physical so that we can make sure everyone is ready to go for the first day of practice.  Thank you for your help in spreading the word.  I hope you are enjoying your summers.

Please have them show up 15 min prior to your scheduled time to fill out paperwork.

The Pima Community College Sports Injury Management staff consists of three full-time athletic trainers who are committed to the well-being and safety of all student-athletes. Potential student-athletes must fill out the Sports Physical Waivers & Consent form and turn it in to the Athletic Trainers on the West Campus in FSS 115.
